Make Your Own Healthcare Decisions
- Reflect about what kind of emergency or life-sustaining treatment you would want if you could not speak for yourself. Who you would want to speak on your behalf.
- Discuss these decisions with your family, friends, or care providers. These conversations can be challenging, but it is important that your wishes are understood completely by those you trust who may be making decisions on your behalf.

Document Your Decisions
Don't have a plan?
- Download our advance directive form. Use it to document your healthcare wishes and appoint a trusted person who can speak for you if you cannot speak for yourself.
- In North Carolina, you must have a notary and two witnesses that meet your state’s legal requirements sign your document. Learn more.
- In South Carolina, you must have a witness that meet your state’s legal requirements sign your document. Learn more.

Already have a plan?
- Great! We are happy that you have taken such an important step to owning your health and advance to the next step.

Upload your plan
- Create a PDF scan of your form. You may only have a single PDF document, though it may have more than one page. The document should be 72dpi or larger. Learn more.
- Login to Mind My Health or create an account if you do not have one.
- Click “Upload Document” on your account dashboard.
- Follow the instructions to complete your upload.

Share your plans
- Download your plan at any time by clicking “Download my Plan” on your account dashboard. Then, you can print or email the PDF document to your family, caregivers, or medical professional.
Note: For supported hospital systems, you may also send your advance care document directly to your health system so that it will be added to your medical record in the Electronic Healthcare Record and can be easily accessed by your care providers if and when needed.

Update Your Plan
- Delete your current plan from your account dashboard by selecting “Delete Document” and following instructions.
- Upload your new plan by selecting “Upload Document” and following instructions.
